According to the U.S. Department of Labor, you must be given at least 15 calendar days to return it, you generally pay for it, and the provider may, but is not required to, list a diagnosis.
FMLA forms are usually best completed by the clinician who is treating the condition over time, often a primary care doctor or specialist. Urgent care may be able to complete forms about the visit itself; ask the clinic about its policy and any fee.
Employer and school rules on when a note is required vary, and some states and cities have their own paid sick leave rules. Check your employee handbook or school policy.
School notes and return-to-school forms
Schools often ask for a note after a child misses several days, or for a clearance note before a child returns to sports after an injury.
